All Coaches must check in at least
60 minutes before their first match. Coaches must present the following items to the onsite check-in tent:
- A Current Spring Season, Coach Photo ID Card
- Copies of each players current Spring Season medical release form.
Please note: Player medical release forms in electronic or digital format, will NOT be accepted. Hard copies MUST be presented at time of check-in.
EARLY Check-In (optional)
- Thursday May 15th @ Outback Steakhouse (Laguna Hills) - 6pm
- Coaches are advised to take advantage of EARLY check-in.
- Coaches must present the items mentioned above (i.e. valid Coach Photo ID Card, valid medical release forms)
Please note: Activated official team rosters from Affinity Sports and four (4) pre-printed registration generated game cards, must be submitted to Event Officials prior to May 5th. Roster and game card file name submission format: DIVISION - REGION - COACH NAME (ROSTER or GAME CARD) [i.e. 14UB-R85-SMITH (roster)]
