Log in to eayso.org
Click on AYSO volunteers
Click on "Print my form or print another copy..."
Click on your name on the next page that comes up to view your volunteer application.
Your AYSO ID number is on the upper right corner of the form.

If you are registering for the first time on eAYSO you may use your pre-registration number in lieu of your permanent AYSO ID to sign up for the class.
